When you want to apply for compensation for a work disability, serious illness or permanent impairment caused by an accident, fill in the claim form.
Save the form to your computer if you plan to send it to us through our Web Service. Or, if you prefer to send it by post, you can print the form.
Once you have filled in the form, you can send it and any attachments (see the required attachments below) via our Web Service attached to a message that concerns the claims matter.
